This pack has been developed to support children in Year 6 with their transition to secondary school. It includes a booklet for children to complete and a parent guide with information on how to support their child to complete the pack.

- identifying similarities and differences between primary school and secondary school
- Saying how they feel about different aspects of going to secondary school
- Understanding and following a weekly timetable for secondary school
- Organisation schools needed for secondary school
- Listening skills that will be needed for secondary school
- Asking for help at secondary school
- Problem solving at secondary school
- Understanding the school map
- Having conversations and making new friends
- Reflecting on the support they will need and creating a communication passport to give to their new teachers
